Ihtibaas-e- Tams (Amenorrhoea)
Introduction
Ihtibaas-e- Tams (Amenorrhoea/ Amenorrhea) is a condition where menstrual cycle stops before time or a condition in which menstrual flow ceases during reproductive life of a women. In few conditions menstrual cycle stops naturally for example before menopause, during pregnancy, after menopause and for a long period after delivery when the mothers are nursing their babies. However, the stoppage of these cycles before time is not normal.
Types of Amenorrhoea: There are two types of amenorrhea
1. Primary Amenorrhoea: It is a condition which takes place when a girl is unable to have her first period by the age of sixteen years due to different reasons such as over exercise, anorexia, stress, ovulation abnormalities, genetic abnormalities, thyroid disorders, pituitary disease or obesity.
2. Secondary Amenorrhoea: It is a condition in which a women who has had her periods but fails to have them again or she is facing irregularities in menstrual cycle along with absent periods.
Causes of Amenorrhoea (Asbaab-e- Ihtibaas-e- Tams)
Some of the important causes of amenorrhoea are given below:

Increased blood viscosity (Ghilzat-e- dam)

Faulty dietary pattern (Ghizayi aadat me nuqs)

Faulty life style (Rahen sahen mein nuqs)

Displacement of uterus

Low physical work

Intake of strong medicines and intoxicants

Obstruction as the openings of uterine vessels

Healed uterine ulcers resulting in vascular obstruction

Stress and anxiety

Imperforated hymen

Diseases like TB, malaria, cough & cold

Malnutrition (Naqs-e- taghzia)

Diseases of liver and spleen

Excess of venesection (Fasd)

Excess of exercise (Riyaazat)

Obesity (Motaapa) which is resulting in pressure on vessels
Ihtibaas-e- Tams ke Alamaat (Symptoms of Amenorrhoea): Stopping of menstrual cycle before time is associated with many signs and symptoms which is characterized by cessation of menses along with specific features of its causative factors. Some important signs and symptoms are:

Hemopoesis (Tawleed-e- Dam) in cases of anaemia

Evacuation of viscous humours (Tanqiya-e- Akhlaat-e- Ghaleeza)

Thinning of humours (Talteef-e- khilt)

Deobstruction of vascular openings (Tafteeh-e- Afwaah-e- Urooq)

Emaciation (Tahzeel) in case of obesity

Surgical intervention is case of imperforated hymen
Ilaj bil- Tadbeer (Regimenal Therapy)

Bloodletting through saphenous vein (Fasd-e- Saafin)

Moistness producing bath (Hammam-e- Murattib)

Cupping on calf (Hijama)
Ilaj bil- Ghiza (Dietotherapy)
Dietary Recommendations: Haemopoetic diet is recommended (Aghziya-e- moallid-e- dam) in case of anaemia (Qillat-e- dam).
Dietary Restrictions: Dietary control (Taqleel-e- Ghiza) is advised in case of obesity
Ilaj bil- Dawa (Pharmacotherapy)

Sitz bath (Aabzan) with decoction of following drugs
Anethum soya (Shibt), Oliganum vulgare Linn. (Marzanjosh), Mentha piperita Linn. (Foodanaj), Ruta graveolens Linn. (Suddab), Matricaria chamomilla Linn. (Baboonah), Pods of Trigonella uncata Linn. (Iklil-ul- malik).
Oral administration of decoction of following drugs

Apium graveolens Linn. (Karafs) and Ziziphora tenuior (Mushktaramasheeá) alongwith honey (Shahed)
-
Mentha piperita Linn. along with Ma-ul- A’sl
-
Ferula asafoetida Linn. (Hilteet) 3.5 gm along with Ma-ul- A’sl
Oral administration of decoction of following drugs along with honey
Apium graveolens Linn. (Karafs), Pimpinella anisum Linn. (Anisoon), Foeniculum vulgare Gaertn (Baadiyan), Mentha arvensis (Pudinah kohi), and Mentha aquatica (Pudinah Nahri)

Pessary (Firzaja) containing following ingredients
Juice of fresh Ruta graveolens Linn. (Aab-e- sudad), juice of fresh Mentha piperita Linn. (Aab-e- pudina)

Health tips for Amenorrhoea patients
There are some health tips, which are important in giving relief from amenorrhoea.

Amenorrhoea patient is advised to take fruits or vegetable juice for three days subject to condition that she shouldn’t be weak. It is better to be on fruits, fruits & vegetable juices for 10 days if her health permits.

Amenorrhoea patient should take sitz bath in the morning and evening.
-
One should try to follow self-discipline natural life.

Wet mud pack on the abdominal region is advisable. Sometimes, steam should be given on the abdomen.

Drinking of lukewarm milk is very beneficial.
-
Consult Unani physician/ expert to learn how to maintain good and holistic health.
-
Always balance diet is recommended in such cases. Check your weight to control obesity at intervals.

Do regular workouts/ exercise moderately.
-
Fruits like papaya, pineapple, pomegranate and apple is advisable.
-
Advisable foods preferably in this condition are pulses, curd, green grams, lentil, onion and sprouted grams
